Guests who would like to participate in Paul Gauguin Cruises’ SCUBA dive programs are required to complete the Discover SCUBA Beginner or Certified Divers forms, which contain a medical questionnaire and liability disclaimer, prior to embarking the m/s Paul Gauguin. Simply, download and print the forms, review and complete them, have your doctor sign the Physician section then bring these aboard the cruise. The doctor on board will not be able to issue the medical certificate. Before signing up for a dive, divers at all levels of experience must meet with one of our dive instructors to review the forms on the first day of the cruise.
